摘要 目的分析慢性阻塞性肺疾病(COPD)合并肺间质纤维化患者外周血及诱导痰上清中炎性细胞因子特点,分析其炎症特征。方法选择2015年7月1日-2017年6月30日在上海市第六人民医院金山分院呼吸科住院的COPD合并肺间质性纤维化患者81例作为观察组,并以单纯COPD患者81例作为对照组。检测并比较2组外周血清及诱导痰上清中炎性细胞因子IL-8、TNF-α、CTGF、TGF-β1及ET-1水平。对81例COPD合并肺间质性纤维化患者进行亚组分析,比较老年亚组与非老年亚组外周血清及诱导痰上清中炎性细胞因子IL-8、TNF-α、CTGF、TGF-β1及ET-1水平。结果观察组外周血中平均CTGF含量高于对照组(P<0.05),诱导痰上清中平均CTGF、TGF-β1及ET-1含量高于对照组(P<0.05)。老年亚组外周血中平均CTGF和ET-1含量高于非老年亚组(P<0.05),诱导痰上清中CTGF、TGF-β1及ET-1平均含量高于非老年亚组(P<0.05)。结论COPD合并肺间质纤维化患者有着独特的肺间质纤维化炎症特征,在老年人中更为显著。 Objective To investigate the clinical characteristics of inflammatory cytokines in peripheral blood and in-duced sputum supernatant of patients with chronic obstructive pulmonary disease(COPD)complicated with pulmonary interstital fibrosis,and analyze its inflammatory characteristics.Methods 81 patients with COPD complicated with pulmonary interstitial fibrosis who were hospitalized in the Department of Respiration of Jinshan Branch from of Sixth People's Hospital of Shanghai City July 1,2015 to June 30,2017 were selected as the observation group,and 81 patients with simple COPD were selected as the control group.The levels of inflammatory cytokines(IL-8,TNF-α,CTGF,TGF-β1 and ET-1)in the peripheral blood and induced sputum supernatant of the two groups were detected and compared.The observation group was further divided into the elderly subgroup and non elderly subgroup.The levels of IL8,TNF-α,CTGF,TGF-β1 and ET-1 in peripheral blood and induced sputum supernatant were compared between the two subgroups.Results The average content of CTGF in peripheral blood of observation group was higher than that of control group(P<0.05),and the average contents of CTGF,TGFB 1 and ET-1 in induced sputum supernatant of observation group was higher than those of control group(P<0.05).The average contents of CTGF and ET-1 in peripheral blood of elderly group were higher than those of non-elderly group(P<0.05),and the average contents of CTGF,TGF-1 and ET-1 in induced sputum supernatant of elderly group were higher than those of non-elderly group(P<0.05).Conclusion COPD patients with pulmonary interstitjal fibrosis have unique inflammatory characteristics of pulmonary interstitial fibrosis,especially in the elderly.
