摘要 目的探讨原发性高血压患者的运动血压与血管细胞黏附分子-1(VCAM-1)、可溶性细胞间黏附分子(sICAM-1)及血管舒张功能(FMD)之间的关系。方法检测80例原发性高血压患者和80名体检健康者(正常对照组)血清VCAM-1、sICAM-1水平。分别测量高血压患者(静息组)和正常对照者静息时的舒张压以及高血压患者中等强度运动后(运动组)的舒张压,根据运动后的舒张压将运动组分为舒张压>11.97 kPa(90 mmHg)和舒张压≤11.97 kPa(90 mmHg)2个亚组。采用Pearson相关分析评估各项指标之间的相关性。结果静息组血清VCAM-1、sICAM-1水平均高于正常对照组(P<0.05)。运动组血清VCAM-1水平高于静息组(P<0.01)。运动组舒张压>11.97 kPa者血清VCAM-1、sICAM-1水平均明显高于舒张压≤11.97 kPa者(P<0.01)。以舒张压代表FMD。正常对照组、静息组及运动组舒张压≤11.97 kPa者的VCAM-1均与FMD呈负相关(r值分别为-0.24、-0.63、-0.52,P<0.05),运动组舒张压>11.97 kPa者VCAM-1与FMD无相关性(r=-0.34,P>0.05);正常对照组sICAM-1与FMD呈正相关(r=0.41,P<0.01),其他各组sICAM-1与FMD均无相关性(P>0.05)。结论原发性高血压患者运动血压与VCAM-1、sICAM-1、FMD密切相关。 Objective To investigate the relationship between exercise blood pressure and vascular cell adhesion molecule-1(VCAM-1),soluble intercellular cell adhesion molecule-1(sICAM-1)and flow mediated dilation(FMD)in patients with essential hypertension.Methods Serum levels of VCAM-1 and sICAM-1 were determined in 80 patients with essential hypertension and 80 healthy subjects(healthy control group).The diastolic blood pressure of hypertensive patients(rest group),healthy control group at rest and hypertensive patients after moderate exercise(exercise group)were measured.According to the diastolic blood pressure after exercise,the exercise group was classified into diastolic blood pressure>11.97 kPa(90 mmHg)and diastolic blood pressure≤11.97 kPa(90 mmHg)groups.Pearson correlation analysis was used to evaluate the correlation between various indicators.Results The levels of serum VCAM-1 and sICAM-1 in rest group were higher than those in healthy control group(P<0.05).Serum VCAM-1 level in exercise group was higher than that in rest group(P<0.01).Serum VCAM-1 and sICAM-1 in exercise group with diastolic blood pressure>11.97 kPa were higher than those with diastolic blood pressure≤11.97 kPa(P<0.01).The diastolic blood pressure represented FMD.The healthy control group,rest group and exercise group had negative correlations between VCAM-1 level and FMD for diastolic blood pressure≤11.97 kPa(r values were-0.24,-0.63 and-0.52,respectively,P<0.05).In exercise group,there was no correlation between VCAM-1 level and FMD in patients>11.97 kPa(r=-0.34,P>0.05).The level of sICAM-1 in healthy control group was positively correlated with FMD(r=0.41,P<0.01).There was no correlation between the level of sICAM-1 and FMD in the other groups(P>0.05).Conclusions Exercise blood pressure in patients with hypertension is closely related to VCAM-1,sICAM-1 and FMD.
