摘要 目的总结并分析短暂性脑缺血病变的三维动脉自旋标记(3D-ASL)、三维时间飞跃法(3D-TOF)及弥散加权成像(DWI)的影像特点。方法回顾性分析昆明医科大学附属延安医院神经内科门诊及住院部2018-08—2020-06收治的短暂性脑缺血发作(TIA)患者43例,所有患者行MRI常规序列检查、DWI、MRA、3D-ASL检查,测量颅内患侧脑实质CBF值,与对侧相应镜像区域CBF比较并进行统计学分析。结果所有患者常规MRI扫描及DWI未见异常,MRA示颅内动脉闭塞者11例,狭窄者25例,7例患者MRA图像无阳性发现,脑灌注减低者30例,与自身对侧对照差异有统计学意义(P<0.05)。结论3D-ASL、MRA多扫描技术联合能够提高TIA的诊断,为临床的诊断、治疗提供客观依据。 Objective To summarize and analyze the imaging characteristics of 3D arterial spin markers(3D-ASL),3D-TOF and dispersion weighted imaging(diffusion weighted imaging,DWI)of transient cerebral isothermic lesions.Methods Totally 43 TIA patients admitted to the Neurology Clinic and Inpatient Department of Yan’an Hospital Affiliated with Kunming Medical University from August 2018 to June 2020 were retrospective analyzed.All patients performed MRI routine sequence examination,DWI,MRA,3D-ASL examination,measured the substantive CBF value of the intracranial side brain,compared with the corresponding mirror re⁃gion CBF on the side,and carried out statistical analysis.Results There were no abnormalities in routine MRI scans and DWI in all patients,11 cases of intracranial artery cleats in MRA,25 cases of narrowing,no positive detection of MRA images in 7 patients,30 cases of reduced cerebral perfusion,and statistically significant differences with their own side control(P<0.05).Conclusion The combined 3D-ASL and MRA multi-scan technology can improve the diagnosis of TIA and provide an objective basis for clinical diag⁃nosis and treatment.
