TAMs与CAFs联合预测子宫颈癌淋巴结转移研究

摘要 目的探讨肿瘤相关巨噬细胞(TAMs)与肿瘤相关成纤维细胞(CAFs)在子宫颈癌组织中的表达分布及其与临床病理参数的相关性,以及二者联合分析对子宫颈癌淋巴结转移的预测意义。方法从广州医科大学附属第一医院病理科组织样本库中收集55例子宫颈癌组织临床样本,利用免疫组织化学法检测子宫颈癌组织病理切片中CD163+TAMs与α-平滑肌肌动蛋白(α-SMA)+CAFs的表达水平,并对免疫组化检测结果与患者临床病理参数的相关性进行统计学分析。结果免疫组化结果显示:CD163+TAMs高度浸润区域往往伴随着α-SMA+CAFs的高表达,二者表达呈正相关(P=0.004)。CD163+TAMs、α-SMA+CAFs的表达分别与子宫颈癌淋巴结转移(P=0.002、P=0.011)和FIGO 2018分期(P=0.014、P=0.007)呈正相关,二者联合表达水平与子宫颈癌的淋巴结转移(P=0.005)和FIGO 2018分期(P=0.005)有显著关联性。ROC曲线可见联合检测方法的曲线下面积(AUC)(0.813)均大于单独检测CD163+TAMs(0.746,P=0.003)、α-SMA+CAFs(0.759,P=0.002),联合检测阳性预测值(PPV)(60%)和阴性预测值(NPV)(94.4%)高于单独检测CD163+TAMs、α-SMA+CAFs的PPV(55.6%、50.0%)和NPV(85.7%、84.0%)。结论子宫颈癌组织中CD163+TAMs与α-SMA+CAFs的表达水平有显著相关性,CD163+TAMs与α-SMA+CAFs的表达水平与子宫颈癌淋巴结转移呈显著正相关,联合检测CD163+TAMs与α-SMA+CAFs可作为子宫颈癌淋巴结转移的前瞻性预测因素。 Objective fibroblasts(CAFs)in cervical cancer tissue and its association with the clinicopathological indexes,and to study the predictive significance of TAMs combined with CAFs in lymph node metastasis of cervical cancer.MethodsThe tissue samples of fifty-five patients with cervical cancer were obtained from the tissue bank of the Department of Pathology,the First Affiliated Hospital of Guangzhou Medical University. Immunohistochemistry was used to determine the expression of CD163+TAMs and α-smooth muscle actin (α-SMA)+CAFs. The immunohistochemical results were analyzed statistically in association with various clinicopathological indexes.ResultsImmunohistochemical staining showed that higher numbers of TAMs expressing CD163 were observed in higher α-SMA+CAFs areas,and there was a positive relationship between them(P=0.004). The expressions of CD163+TAMs and α-SMA+CAFs were positively correlated with lymph node metastasis(P=0.002,P=0.011)and FIGO 2018 stage(P=0.014,P=0.007)of cervical cancer,and the combined expression of the two was significantly related to lymph node metastasis of cervical cancer(P=0.005)and FIGO2018 stage(P=0.005). The ROC curve showed that the area under the curve(AUC)of the joint detection method(0.813)was larger than that of separate detection for CD163+TAMs(0.746,P=0.003)and α-SMA+CAFs(0.759,P=0.002),and the positive predictive value(PPV=60%)and negative predictive value(NPV=94.4%)of combined detection were higher than PPV(55.6%,50.0%)and NPV(85.7%,84.0%)ofseparate detection for CD163+TAMs and α-SMA+CAFs.ConclusionThere is a significant correlation betweenthe expression of CD163+TAMs and that of α-SMA+CAFs in cervical cancer,and there is a significantpositive correlation between their expression and lymph node metastasis of cervical cancer. The combined detection of CD163+TAMs and α-SMA+CAFs can be used as aprospective predictor of lymph node metastasis of cervical cancer.
