经食管三维超声心动图在人工主动脉瓣瓣周漏诊断与定量评估中的应用

摘要 目的探讨3D-TEE在人工主动脉瓣瓣周漏诊断和定量评估中的应用价值。方法选取于我院就诊,TTE诊断为主动脉瓣瓣周漏并行3D-TEE检查的患者40例为研究对象,对瓣周漏的数量、位置、形状定性评估,对其大小及反流情况进行定量测量,对比两种检查方法的不同结果。结果 40例患者行3D-TEE后33例确诊为主动脉瓣瓣周漏,其中26例为单束瓣周漏,7例具有2束及以上瓣周漏;轻度13例、中度11例、重度9例。与TTE相比,3D-TEE测得的瓣周反流缩流径宽度及周向范围较大,通过多平面重建可以测量瓣周漏的长度、宽度及反流口面积。结论 3D-TEE能够精确显示主动脉瓣瓣周漏的数量、位置及形态,定量评估其大小及严重程度,在瓣周漏的临床诊断和术前评估中具有重要价值。 Objective To assess the utility of 3D-TEE in the diagnosis and quantitative evaluation of prosthetic aortic paravalvular leak. Methods 40 patients who were diagnosed with aortic paravalvular leak by TTE and underwent 3D-TEE were included. The number, location and shape of the paravalvular leaks were qualitatively evaluated, the size and severity were measured. The results of the two examinations were compared. Results Among the 40 patients who underwent 3D-TEE, 33 of them were diagnosed with aortic paravalvular leak, of which 26 cases were single-beam paravalvular leak, and 7 cases had 2 or more beams;13 cases were mild, 11 cases were moderate, and 9 cases were severe. Compared with TTE, the vena contracta and circumferential range of paravalvular leak measured by 3D-TEE were larger. In addition, the length, width of paravalvular leak and regurgitant orifice area can also be measured. Conclusions 3D-TEE provided detailed descriptions of the number, location and shape of aortic paravalvular leak, quantitatively evaluated the size and severity, which has significant value in clinical diagnosis and preoperative evaluation of prosthetic aortic paravalvular leak.
