摘要 目的探究改良硬膜外导管连接装置持续腰大池引流术辅助治疗颅内感染的临床效果。方法选取2016年1月-2018年1月中南大学湘雅医学院附属海口医院收治的颅内感染患者49例,按照随机数表法分为研究组(28例)和对照组(21例),研究组接受改良硬膜外导管连接装置持续腰大池引流术治疗,对照组接受传统硬膜外导管连接装置持续腰大池引流术治疗。记录两组患者治疗期间引流情况,测定治疗前后脑脊液中多核细胞比例、葡萄糖、氯化物和蛋白质水平。酶联免疫吸附法测定治疗前后血清C-反应蛋白(CRP)和降钙素原(PCT)含量,血常规测定全血白细胞数和中性粒细胞比例。结果研究组总不良事件低于对照组(P<0.001);治疗前后,研究组多核细胞比例差值和蛋白质差值、氯化物差值、葡萄糖差值高于对照组(均P<0.001);治疗前后,研究组白细胞数差值和中性粒细胞比例差值、CRP差值、PCT差值高于对照组(均P<0.001)。结论改良硬膜外导管连接装置持续腰大池引流术治疗颅内感染效果显著,值得临床使用。 OBJECTIVE To explore the clinical effect of continuous lumbar cistern drainage with modified epidural catheter connection device on adjuvant treatment of intracranial infection.METHODS A total of 49 patients with intracranial infection who were treated in Central South University Xiangya School of Medicine Affiliated Haikou Hospital from Jan 2016 to Jan 2018 were enrolled in the study and randomly divided into the study group with 28 cases and the control group with 21 cases.The study group was treated with continuous lumbar cistern drainage with modified epidural catheter connection device,while the control group was treated with the continuous lumbar cistern drainage with conventional epidural catheter connection device.The status of drainage was recorded during the treatment period,the percentages of cerebrospinal fluid multinucleated cells,glucose,chloride and protein were determined,the levels of serum C-reactive protein(CRP)and procalcitonin(PCT)were determined by using enzyme-linked immunosorbent assay before and after the treatment,and the percentages of whole blood white blood cell and neutrophils were detected by means of blood routine examination.RESULTS The total incidence of adverse reactions of the study group was significantly lower than that of the control group(P<0.001).The difference values of multinucleated cell percentage,difference values of chloride and glucose of the study group were significantly higher than those of the control group before and after the treatment(all P<0.001).The difference values of white blood cell counts,neutrophils,CRP and PCT of the study group were significantly higher than those of the control group before and after the treatment(all P<0.001).ConCLUSION The continuous lumbar cistern drainage with modified epidural catheter connection device can achieve significantly effect on treatment of intracranial infection,and it is worthy to be promoted in the hospital.
