摘要 目的观察左氧氟沙星滴眼液治疗细菌性角膜炎的效果、安全性与成本—效果分析。方法选取2018年10月-2019年9月大连市金州区第三人民医院收治的细菌性角膜炎患者126例,采用随机数字表法将患者分为观察组和对照组,各63例。对照组应用氧氟沙星滴眼液治疗,观察组应用盐酸左氧氟沙星滴眼液治疗。比较2组患者临床症状(结膜充血水肿、角膜浸润性疼痛与畏光流泪)缓解时间,即治疗开始至症状显著好转所需时间;比较2组不良反应发生情况,并根据其用药成本(C,元)与治疗效果(E,%)计算比较2组成本—效果分析结果(C/E)。结果观察组治疗总有效率为93.6%,高于对照组的73.0%(χ2=11.402,P=0.001);观察组结膜充血水肿、角膜浸润性疼痛与畏光流泪等临床症状缓解时间均明显短于对照组(P <0.01);2组不良反应总发生率比较差异无统计学意义(P>0.05);观察组C/E为26.7,高于对照组的16.4。结论左氧氟沙星滴眼液治疗细菌性角膜炎成本较氧氟沙星滴眼液高,但其临床疗效更佳,药物安全性高,可迅速缓解患者临床症状,促进病情好转,更具临床应用推广价值。 Objective To observe the efficacy,safety and cost-effectiveness of levofloxacin eye drops in the treatment of bacterial keratitis. Methods One hundred and twenty-six patients with bacterial keratitis admitted to the third people’ s Hospital of Jinzhou District of Dalian city from October 2018 to September 2019 were selected and randomly divided into observation group and control group,63 cases in each group. The control group was treated with ofloxacin eye drops,and the observation group was treated with levofloxacin hydrochloride eye drops. The remission time of clinical symptoms( conjunctival congestion and edema,corneal invasive pain and photophobic tears) was compared between the two groups;The incidence of adverse reactions was compared between the two groups,and the cost-effectiveness analysis results( C/E) were calculated and compared according to the drug cost( C,Yuan) and treatment effect( E,%). Results The total effective rate of the observation group was 93. 6%,which was higher than 73. 0% of the control group( χ2= 11. 402,P = 0. 001);The remission time of conjunctival congestion and edema,corneal invasive pain,photophobia and tears in the observation group was significantly shorter than that in the control group( P < 0. 01);There was no significant difference in the total incidence of adverse reactions between the two groups( P > 0. 05);The C/E of the observation group was 26. 7,which was higher than 16. 4 of the control group. Conclusion The cost of levofloxacin eye drops in the treatment of bacterial keratitis is higher than that of ofloxacin eye drops,but it has better clinical efficacy and high safety. It can quickly relieve the clinical symptoms of patients,promote the improvement of the condition,and has more clinical application value.
