摘要 目的:探讨益肾降浊冲剂对5/6肾切除大鼠肾组织纤维化的影响及其机制。方法:利用大鼠5/6肾切除模型,将大鼠随机分为假手术组、模型组、益肾降浊冲剂低、中、高剂量组和大黄素组,每组10只。益肾降浊冲剂按照低、中、高剂量(9 g/kg,18 g/kg,36 g/kg)配成水煎剂,大黄素的剂量为500 mg/kg,灌胃量为1 mL/100 g,模型组和假手术给予等体积蒸馏水灌胃,连续用药8周。治疗结束后,取腹主动脉血检测各组大鼠尿素氮(BUN)、肌酐(SCR);ELISA法检测各组大鼠血清中白细胞介素1β(IL-1β)、白细胞介素6(IL-6)、肿瘤坏死因子α(TNF-α)含量;Masson染色观察肾脏纤维化程度;免疫组化法检测肾组织核因子κB(NF-κB)、转化生长因子β1(TNF-β1)及Smad3蛋白含量。结果:与假手术组相比,模型组大鼠血清BUN、SCR水平升高(P<0.01),肾组织纤维化程度加重,血清中IL-1β、IL-6、TNF-α水平升高(P<0.05,P<0.01),NF-κB、TGF-β1、Smad3蛋白表达升高(P<0.05,P<0.01)。与模型组相比,益肾降浊冲剂各剂量组大鼠血清BUN、SCR水平下降(P<0.01),肾组织纤维化程度减轻,血清中IL-1β、IL-6、TNF-α水平降低(P<0.05,P<0.01),NF-κB、TGF-β1、Smad3表达下降(P<0.05,P<0.01)。结论:益肾降浊冲剂可以改善尿毒症大鼠的肾脏功能,减轻肾脏纤维化,其机制可能与下调NF-κB和TGF-β1/Smad3通路进而改善尿毒症大鼠肾纤维化有关。 Objective:To investigate the effect of Yishen Jiangzhuo granule on renal fibrosis in 5/6 nephrectomized rats and its effect mechanism.Methods:Using 5/6 nephrectomy model,rats were randomly divided into sham operation group,model group,Yishen Jiangzhuo granule low,medium and high dose groups and emodin group,10 rats in each group.Yishen Jiangzhuo granule was prepared with low,medium and high doses(9 g/kg,18 g/kg,36 g/kg).Emodin was 500 mg/kg,and the intragastric dose was 1 mL/100 g.The model group and sham operation group were given equal volume of distilled water for 8 weeks.After treatment,the serum levels of BUN,SCR were measured by abdominal aortic blood,IL-6,TNF-α,TGF-β1 and Smad3 were detected by immunohistochemistry.Results:Compared with sham operation group,the serum levels of BUN and SCR in model group increased(P<0.01),renal fibrosis was aggravated,and the levels of IL-1β,IL-6,TNF-αin serum increased(P<0.05,P<0.01),NF-κB,TGF-β1,Smad3 protein expression increased(P<0.05,P<0.01);Compared with the model group,the levels of BUN and SCR in serum of rats in Yishen Jiangzhuo granule group decreased(P<0.01),the degree of renal fibrosis was reduced,the levels of IL-1β,IL-6,TNF-αin serum decreased(P<0.05,P<0.01),NF-κB,TGF-β1 and Smad3 expression decreased(P<0.05,P<0.01).Conclusion:Yishen Jiangzhuo granule can improve renal function and reduce renal fibrosis in uremic rats,and its mechanism may be related to down regulating NF-κB and TGF-β1/Smad3 pathway to improve renal fibrosis in uremic rats.
