摘要   目的  通过调查某市二、三级医院住院病案首页现状，分析影响首页数据质量的原因，提出干预措施。 方法  依据国家卫计委《住院病案首页数据填写质量规范（暂行）》等文件规定，对某市2016年9月——2017年2月共750份住院病案首页进行统计分析。 结果  全市三级医院7家，二级医院18家，共抽查750份病案，存在缺陷405份，缺陷率54%。首页存在问题个数943个，平均问题个数为2.33个，其中三级医院首页平均问题个数1.76个，二级医院首页平均问题个数为2.36个。结论  建立住院病案首页专项质控体系，加强临床医生和编码员培训，有效沟通，多部门联动，完善信息系统建设，构建信息共享平台是提高住院病案首页数据质量的有效途径。

Current situation analysis and Countermeasure of inpatient medical record homepage in two or three grade hospital of a city   Zhu Jin Li， Department of medical record management; Affiliated Hospital of Yangzhou University

[Abstract ]    Objective  By investigating the status quo of the first page of inpatient medical record in two or three grade hospital of a city, the paper analyzes the reasons for influencing the data quality of the front page and puts forward some intervening measures. Methods a total of 750 inpatient medical records in September 2016 and February 2017 were analyzed statistically according to the documents stipulated by the State Health Planning Commission, "the first page of inpatient medical records and the quality specification (Interim)". Results there were 7 hospitals in grade three and two hospitals in grade 18. A total of 750 medical records were selected, 405 defects, and the defect rate was 54%. There are 943 problems in the home page, the average number of problems is 2.33, of which the average number of hospitals in three families is 1.76, and the average number of questions in two hospitals is 2.36. Conclusion the establishment of medical records quality control system, strengthen the training of clinicians and encoding, effective communication, multi sectoral linkage, perfect information system construction, build the information sharing platform is an effective way to improve the quality of hospital medical record data.

随着DRGs在我国逐步试点和推广应用，住院病案首页的重要性日益凸显。住院病案首页是整份病案的窗口，浓缩了病历中各项内容的精华。DRGs分组所需的信息几乎均来自于住院病案首页。因此，住院病案首页信息质量受到多方强烈关注，成为评价医院临床服务能力和医疗质量水平、实现医院精细化管理的核心医疗数据^[1]。 为了了解某市二、三级医院住院病案首页的质量情况，某市卫生和计划生育委员会组织专家，于2017年3月21日——27日对某市25家二、三级医院进行住院病案首页的专项检查。

资料与方法

1、研究对象

某市下辖邗江区、广陵区、江都区3个区和宝应县1个县以及高邮市、仪征市2个县级市，以县市级医院为主，其中三级医院7家，二级医院18家。

2、检查内容

本次检查依据国家卫计委于2016年5月31日下发的《住院病案首页数据填写质量规范（暂行）》和《住院病案首页数据质量管理与控制指标（2016版）》的通知，重点检查如下内容：主要诊断、其他诊断、主要手术及操作、其他手术及操作、入院病情、新生儿出生体重、新生儿入院体重、重症监护时间、呼吸机使用时间、入院前昏迷时间、入院后昏迷时间、离院方式等项目。

3、检查方法

采用随机抽样的方法，从全市2016年9月-----2017年2月半年以内的出院病案中调取，每家医院抽取30份，要求：. ①住院7-----30天以内的内科病历10份，. ② 三级医院手术科室选取3级手术以上的病历，二级医院手术科室选取2级手术以上的病历10份，③. 死亡病历5份，④.不足部分随机抽取。此次检查采用专家组现场检查的方式，检查组每组2----3人，调取已归档的病案进行疑似问题追踪。根据住院病案首页数据质量评分表，把住院病案首页信息分成四大类37项，四大类包括患者基本信息、住院过程信息、诊疗信息、费用信息。通过认真阅读每份病历的入院记录、出院记录、检查化验单、手术记录、病理记录、死亡讨论等信息对住院病案首页中的主要诊断、主要手术操作、其他诊断、其他手术操作的选择、编码的准确性以及新生儿出生体重、新生儿入院体重等的完整性、正确性进行判断。
